Vital Signs Quick Reference

Normal ranges, critical values & nursing pearls — CNA/LPN/RN

Vital Sign
✓ Normal
⚠ Abnormal
🚨 Critical
💡 Nursing Pearl
🫀Blood Pressure
120/80 mmHg
↓ < 90/60 mmHg
↑ > 140/90 mmHg
🚨 > 180/120 mmHg

Hypotensive: Hold meds, notify HCP. Hypertensive: Check for headache, vision changes.

💓Heart Rate
60–100 bpm
↓ < 60 bpm (Bradycardia)
↑ > 100 bpm (Tachycardia)
🚨 < 40 or > 150 bpm

Always assess rhythm, not just rate. Check apical pulse for 1 full minute.

🫁Respiratory Rate
12–20 breaths/min
↓ < 12 (Bradypnea)
↑ > 20 (Tachypnea)
🚨 < 8 or > 30 breaths/min

Count for full 60 sec. RR is most sensitive early indicator of deterioration.

🌡️Temperature
97.8–99.1°F (36.5–37.3°C)
↓ < 96.8°F (Hypothermia)
↑ > 100.4°F (Fever)
🚨 > 104°F or < 95°F

Rectal most accurate. Axillary reads 1°F lower. Oral reads 1°F lower than rectal.

💨Oxygen Saturation
95–100%
↓ 91–94% (Mild hypoxia)
🚨 < 90% (Emergency)

Apply O₂ if SpO₂ < 94%. Nail polish, poor perfusion can cause false readings.

😣Pain Scale
0 (No pain)
↓ 1–3 (Mild)
↑ 7–10 (Severe)
🚨 Uncontrolled pain → assess cause

Reassess 30 min after intervention. Document location, quality, radiation, timing.

📌

Always report critical values immediately. Document time, value, who was notified, and response.
